Attend an employee meeting where you will receive your eligibility form and your enrollment packet.

October 31

First day to complete the online HRA to qualify for the Healthy Rewards Rate in 2012.

December 31

Deadline for submitting dependent documentation and the other coverage surcharge affidavit as well as completing the online Health Risk Assessment.

January 1, 2012

New benefit elections take effect.
