When people leave traditional hospitals they often need continued care to recover completely. That’s where we come in. Kindred’s expertise across a variety of post-acute care sites of service helps us provide care for patients in the most appropriate setting.
We specialize in delivering quality medical interventions and successfully transitioning patients home or to a less intense level of care that meets their needs and enhances their quality of life.In the Cleveland area, Kindred offers services including aggressive care for medically complex patients, intensive care and short-term rehabilitation through two transitional care hospitals, a subacute unit, three nursing and rehabilitation centers, home health and assisted living services.
Our transitional care hospitals (certified as long-term acute care hospitals) provide specialized, goal-directed care for critically ill patients requiring extended recovery time. Licensed as acute care hospitals, we have an additional Medicare certification that supports a length of stay measured in weeks as compared to the typical five-day stay for patients in traditional hospitals. Our specialized care includes: pulmonary care (ventilator management and weaning), complex wound care, dialysis, IV antibiotic therapy and pain management. We offer 24-hour physician support and the ancillary services found at an acute care hospital, including laboratory, radiology, operating and procedure rooms, special care units and telemetry units.
Situated within a transitional care hospital, our Subacute Unit is designed to successfully transition patients to a less intense level of service, such as a skilled nursing center, or home with additional services. We provide inpatient medical care and rehabilitation for those patients with an acute illness, injury or exacerbation of a disease who no longer require the high intensity, aggressive medical care provided in a hospital. Being co-located within a transitional care hospital enables care and treatment by the same physicians, therapists and care professionals, helping to ensure a coordinated transition and improved outcomes.
From specialized short-term, inpatient rehabilitation to compassionate long-term care, our centers provide a full range of medical, rehabilitative and social services to treat and support each of our patients. Our goal is to help patients achieve positive outcomes, regain function and safely return home as quickly as their recovery allows. Services include physical, occupational and speech language therapy, skilled nursing, dementia care and specialized short-term rehabilitative programs that address a wide variety of conditions including pulmonary, cardiac, wound care, orthopedic and stroke recovery.
Kindred Assisted Living Centers offer a full range of services, from housekeeping to restaurant-quality dining, that help residents remain independent. Apartments are designed to provide privacy with easy access to activities and amenities. The care we deliver includes nursing, pharmacy, dietary and social services, as well as a range of additional services to enhance our residents’ quality of life. Residents enjoy a vibrant community and companionship, as well as the comforts of home, with less worry about day-to-day needs.
The home health professionals with Kindred at Home provide medical care and services – including nursing care, wound care and rehabilitation therapies – delivered in the comfort of a patient’s own home. Experienced nurses, therapists and aides work with each individual to maximize physical abilities, improve health and well-being, assist with a variety of daily activities including bathing, and to provide essential education and management of medications and medical conditions.
Our physicians, nurse practitioners and nurses provide compassionate medical care in the homes and assisted and independent living facilities of Ohio seniors. Our board-certified physicians direct complete primary care, specialty care and urgent care services for people who cannot leave home to get to doctor's visits.
With the capability to coordinate all aspects of medical care for individuals in the home, hospital, or nursing home, our patients receive outstanding care while developing a long-term relationship with their care providers. We provide complete complex chronic and acute care management for our patients. We also offer attending hospital physicians, assistance in care transitions, dementia care, palliative care and podiatric care.
We also provide personal home care assistance because we know that caring for our patients doesn’t end with their medical needs. This care may include personal hygiene assistance, meal preparation, transportation, companionship and respite care for family caregivers in the individual’s home.
Kindred, through RehabCare, offers a variety of rehabilitation services including physical, occupational and speech-language therapies. These services are offered across the entire post-acute continuum through every transition.
We are proud to offer an innovative, new approach to care through our Care Transitions Program. The program is a quality improvement initiative designed for patients with medically complex conditions. We provide patients with Care Transitions Managers, patient advocates and navigators who communicate directly with patients, caregivers and primary care physicians or specialists every step of the way.
