That’s where we come in. Kindred’s expertise across a variety of post-acute care sites of service helps us provide care for patients in the most appropriate setting.
At Kindred we specialize in delivering quality medical interventions and successfully transitioning patients home or to a less intense level of care that meets their needs and enhances their quality of life.In the Dallas/Fort Worth area, Kindred offers services including aggressive, medically complex care, intensive care and short-term rehabilitation in seven transitional care hospitals, one subacute unit, one inpatient rehabilitation hospital, three transitional care and rehabilitation centers, outpatient services and home health and hospice care.
Our transitional care hospitals (certified as long-term acute care hospitals) provide specialized, goal-directed care for critically ill patients requiring extended recovery time. Licensed as acute care hospitals, we have an additional Medicare certification that supports a length of stay measured in weeks as compared to the typical five-day stay for patients in traditional hospitals. We offer 24-hour physician support and the ancillary services found at an acute care hospital, including laboratory, radiology, operating or procedure rooms, special care units and telemetry units. Our specialized care includes: pulmonary care (ventilator management and weaning), complex wound care, dialysis, IV antibiotic therapy and pain management.
Through expert, intense interdisciplinary care and aggressive therapy, our free standing Inpatient Rehabilitation Hospitals and hospital-based Acute Rehabilitation Units – all certified as Inpatient Rehabilitation Facilities (IRFs) by the Centers for Medicare and Medicaid Services (CMS) – are designed to provide rapid recovery and improved function for patients who can tolerate at least three hours of rehabilitative care per day. Additionally, these settings provide physician-supervised interdisciplinary care for patients who have a clinical need for therapy in at least two disciplines, daily physician oversight and 24-hour nursing care.
Situated within our transitional care hospital, our Subacute Unit is designed to successfully transition patients to a less intense level of service, such as a skilled nursing center, or home with additional services. The unit provides comprehensive inpatient medical care and rehabilitation for those patients with an acute illness, injury or exacerbation of a disease who no longer require the high intensity, aggressive medical care provided in a hospital. Being co-located with our transitional care hospital enables care and treatment by the same physicians, therapists and care professionals, helping ensure a coordinated transition and improved outcomes.
From specialized short-term, inpatient rehabilitation to compassionate long-term care, our centers provide a full range of medical, rehabilitative and social services to treat and support each of our patients. Our goal is to help patients achieve positive outcomes, regain function and safely return home as quickly as their recovery allows. Our services include physical, occupational and speech language therapy, skilled nursing, dementia care and specialized short-term rehabilitative programs that address a wide variety of conditions.
The home health professionals with IntegraCare, an affiliate of Kindred at Home, provide medical care and services – including nursing care, wound care and rehabilitation therapies – delivered in the comfort of a patient’s own home. Experienced nurses, therapists and aides work to maximize physical abilities, improve health and well-being and provide essential education and management of medications and medical conditions.
Our hospice services provide a family-oriented model of care designed to meet the spiritual, emotional and physical needs of patients in life’s final transition. Hospice care focuses on living the remainder of life comfortably, free of pain and with dignity, and avoiding return to the hospital. Under the guidance of a physician, our hospice care provides essential respite care for family caregivers while keeping the patient in a familiar and comfortable environment. Patient and family support and education are central to our hospice care, and bereavement services are offered for the family up to one year after the patient passes.
We also provide personal home care assistance because we know that care for our patients doesn’t stop with their medical needs. This type of care may include meal preparation, transportation, companionship and respite care for family caregivers in the home.
Kindred, through RehabCare, offers a variety of rehabilitation services including physical, occupational and speech-language therapies. These services are offered across the entire post-acute continuum through every transition.
We are proud to offer an innovative, new approach to care through our Care Transitions Program. The program is a quality improvement initiative designed for patients with medically complex conditions. We provide patients with Care Transitions Managers, patient advocates and navigators who communicate directly with patients, caregivers and primary care physicians or specialists every step of the way.
