That’s where we come in. Kindred’s expertise across a variety of post-acute care sites of service helps us provide care for patients in the most appropriate setting.
In the Dallas/Fort Worth area Kindred offers services including aggressive, medically complex care, intensive care and short-term rehabilitation in seven transitional care hospitals, one subacute unit, one inpatient rehabilitation hospital, three transitional care and rehabilitation centers, outpatient services and home health and hospice care.
At Kindred we specialize in delivering quality medical interventions and successfully transitioning patients home or to a less intense level of care that meets their needs and enhances their quality of life.
Our transitional care hospitals (certified as long-term acute care hospitals) provide specialized, goal-directed care for critically ill patients requiring extended recovery time. Licensed as acute care hospitals, we have an additional Medicare certification that supports a length of stay measured in weeks as compared to the typical five-day stay for patients in traditional hospitals. We offer 24-hour physician support and the ancillary services found at an acute care hospital, including laboratory, radiology, operating or procedure rooms, special care units and telemetry units. Our specialized care includes: pulmonary care (ventilator management and weaning), complex wound care, dialysis, IV antibiotic therapy and pain management.
Our Inpatient Rehabilitation Hospital offers specialized inpatient programs that provide ongoing care. These hospitals are for individuals who have been impaired by an injury or illness and are considered by their physicians to be medically stable and physically able to begin a comprehensive rehabilitation program consisting of at least three hours of therapy a day, five days a week.
Situated within our transitional care hospital, our Subacute Unit is designed to successfully transition patients to a less intense level of service, such as a skilled nursing center, or home with additional services. The unit provides comprehensive inpatient medical care and rehabilitation for those patients with an acute illness, injury or exacerbation of a disease who no longer require the high intensity, aggressive medical care provided in a hospital. Being co-located with our transitional care hospital enables care and treatment by the same physicians, therapists and care professionals, helping ensure a coordinated transition and improved outcomes.
From specialized short-term, inpatient rehabilitation to compassionate long-term care, our centers provide a full range of medical, rehabilitative and social services to treat and support each of our patients. Our goal is to help patients achieve positive outcomes, regain function and safely return home as quickly as their recovery allows. Our services include physical, occupational and speech language therapy, skilled nursing, dementia care and specialized short-term rehabilitative programs that address a wide variety of conditions.
Our home health services provide medical care delivered in the comfort of a qualified patient’s home, focusing on improving independence and reducing rehospitalization. Services are guided by a plan of care established and reviewed regularly with the patient’s physician. Experienced nurses, therapists and aides work with each individual to maximize physical abilities and improve health and well-being by providing skilled nursing care and, where indicated, wound care and physical and occupation therapies. Education is provided to help patients manage their medications and medical conditions. Our hospice services provide a family-oriented, interdisciplinary model of care designed to meet the spiritual, emotional and physical needs of qualified patients with life-limiting conditions who no longer respond to curative treatments. Hospice care focuses on living the remainder of life comfortably, free of pain and with dignity, and avoiding return to the hospital. Hospice care is delivered wherever the patient considers home by a team that includes a nurse, home health aide, social worker, chaplain, and other caregivers as indicated by the plan of care. Patient and family support and education are central to our hospice care, and bereavement services are offered for the family up to 13 months after the patient passes.
Kindred, through RehabCare, offers a variety of rehabilitation services including physical, occupational and speech-language therapies. These services are offered across the entire post-acute continuum through every transition.
