At Kindred we specialize in delivering quality medical interventions and successfully transitioning patients home or to a less intense level of care that meets their needs and enhances their quality of life.In the Louisville and Southern Indiana area, Kindred offers services including aggressive
care for medically complex patients, intensive care and short-term rehabilitation through
two transitional care hospitals, a subacute unit, six nursing and rehabilitation centers, an
outpatient wound care unit, home health and hospice services.
Our Kentuckiana transitional care hospitals (certified as long-term acute care hospitals) are unique in their ability to care for critically ill patients who require specialized, aggressive, goal-directed care over an extended recovery time. Our hospitals are licensed as acute care hospitals, but have an additional Medicare certification that supports a length of stay measured in weeks, not days, as compared to the typical five-day stay for patients in traditional hospitals. We offer specialized services and extended care for medically complex patients who are unable to recover completely in the short-term setting. Our hospitals have 24-hour physician support and the ancillary services found at an acute care hospital, including laboratory, operating and procedure rooms, radiology, special care units and telemetry units. Our specialized care includes: pulmonary care (ventilator management and weaning), medically complex patients (i.e. complex wound care, cardio-thoracic, gastro-intestinal, liver and diabetic conditions, malignant or end stage disease), dialysis, IV antibiotic therapy, cardiac IV therapy, heart failure IV therapy and pain management.
Situated within Kindred Hospital Louisville, our Subacute Unit is designed to successfully transition patients to a less intense level of service, such as a skilled nursing center. The unit provides comprehensive inpatient medical care and rehabilitation. We offer an exclusive service for those patients who require 12 plus hours of continuous ventilator support. Our goal is to wean patients from the ventilator whenever possible, or discharge them with a home ventilator, closely monitored by family members. Being co-located enables care and treatment by the same physicians, therapists and care professionals, helping ensure a coordinated transition and improved outcomes.
From specialized short-term, inpatient rehabilitation to compassionate long-term care, our Kentuckiana centers provide a full range of medical, rehabilitative and social services to treat and support each of our patients. Our goal is to help patients achieve positive outcomes, regain function and safely return home as quickly as their recovery allows. Our services include physical, occupational and speech language therapy, skilled nursing, dementia care and specialized short-term rehabilitative programs that address a wide variety of conditions including pulmonary, cardiac, wound care, orthopedic and stroke recovery.
The home health professionals provide high-quality, comprehensive and compassionate care in the comfort of the patient’s home. We deliver services according to a plan of care that is developed by the patient, family members and physician to encourage independent functioning. Our in-home services allow each patient to remain safe in familiar, comfortable surroundings with treatment delivered by expertly trained nurses, therapists, social workers and aides.

Through Gentiva, an affiliate of Kindred at Home, the professionals in Kentuckiana’s
hospice locations provide family oriented care in order to meet the physical, emotional and spiritual needs of both terminally ill patients and their families. Our services allow patients to stay in a familiar environment while providing support such as pain management, aggressive comfort measures and psychological support in order to provide the best possible end-of-life experience.
Kindred, through RehabCare, offers a variety of rehabilitation services including physical, occupational and speech-language therapies. These services are offered across the entire post-acute continuum through every transition.
