At Kindred we specialize in delivering quality medical interventions and successfully transitioning patients home or to a less intense level of care that meets their needs and enhances their quality of life.
In the Los Angeles area, Kindred offers services including aggressive, medically complex care, intensive care and short-term rehabilitation in 13 transitional care hospitals, one subacute unit, two transitional care and rehabilitation centers, home health, hospice and personal home care services.
Our transitional care hospitals (certified as long-term acute care hospitals) provide specialized, goal-directed care for critically ill patients requiring extended recovery time. Licensed as acute care hospitals, we have an additional Medicare certification that supports a length of stay measured in weeks as compared to the typical five-day stay for patients in traditional hospitals. We offer 24-hour physician support and the ancillary services found at an acute care hospital, including laboratory, radiology, operating or procedure rooms, special care units and telemetry units. Our specialized care includes: pulmonary care (ventilator management and weaning), complex wound care, dialysis, IV antibiotic therapy and pain management.
Situated within our transitional care hospital, our Subacute Unit is designed to successfully transition patients to a less intense level of service, such as a skilled nursing center, or home with additional services. The unit provides comprehensive inpatient medical care and rehabilitation for those patients with an acute illness, injury or exacerbation of a disease who no longer require the high intensity, aggressive medical care provided in a hospital. Being co-located with our transitional care hospital enables care and treatment by the same physicians, therapists and care professionals, helping ensure a coordinated transition and improved outcomes.
From specialized short-term, inpatient rehabilitation to compassionate long-term care, our centers provide a full range of medical, rehabilitative and social services to treat and support each of our patients. Our goal is to help patients achieve positive outcomes, regain function and safely return home as quickly as their recovery allows. Our services include physical, occupational and speech language therapy, skilled nursing, dementia care and specialized short-term rehabilitative programs that address a wide variety of conditions.
The home health professionals with Kindred at Home and Gentiva, an affiliate of Kindred at Home, provide medical care and services – including nursing care, wound care and rehabilitation therapies – delivered in the comfort of a patient’s own home. Experienced nurses, therapists and aides work with each individual to maximize physical abilities, improve health and well-being, assist with a variety of daily activities including bathing, and to provide essential education and management of medications and medical conditions.
Our professionals at our hospice locations provide a family oriented care model in order to meet the physical, spiritual and emotional needs of terminal patients and their families. Our services allow patients to stay in a comfortable environment while providing support such as pain management, aggressive comfort measures and psychological support to create the best possible end-of-life experience.
We also provide personal home care assistance because we know that care for our patients doesn’t stop with their medical needs. This type of care may include meal preparation, transportation, companionship and respite care for family caregivers in the home.
Kindred, through RehabCare, offers a variety of rehabilitation services including physical, occupational and speech-language therapies. These services are offered across the entire post-acute continuum through every transition.
