At Kindred Healthcare we understand that when people are discharged from short-term acute care hospitals they often need continued care to recover completely and successfully. That’s where we come in. Kindred’s expertise across a variety of post-acute care sites of service helps us provide care for patients in the most appropriate setting.
We specialize in delivering quality medical interventions and transitioning patients to the lowest cost setting that meets their health needs and enhances their quality of life.
In the St. Louis area, Kindred offers services including aggressive, medically complex care, intensive care and short-term rehabilitation in three transitional care hospitals, one inpatient rehabilitation facility, home health and hospice services.
Our transitional care hospitals (certified as long-term acute care hospitals) in St. Louis are unique in their ability to care for critically ill patients who require specialized, aggressive, goal-directed care over an extended recovery time. Our hospitals are licensed as acute care hospitals, but have an additional Medicare certification that supports a length of stay measured in weeks not days as compared to the typical five-day stay for patients in traditional hospitals. We offer specialized services and extended care for medically complex patients who are unable to recover completely in the short-term setting. Our hospitals have 24-hour physician support and the ancillary services found at an acute care hospital, including laboratory, radiology, operating or procedure rooms, special care units and telemetry units. Our specialized care includes: pulmonary care (ventilator management and weaning), complex wound care, dialysis, IV antibiotic therapy and pain management.
Through expert, intense interdisciplinary care and aggressive therapy, our free standing Inpatient Rehabilitation Hospital - certified as Inpatient Rehabilitation Facilities (IRFs) by the Centers for Medicare and Medicaid Services (CMS) – seeks to provide rapid recovery and improved function for patients who require intense rehabilitation and physician-supervised interdisciplinary care. These hospitals are for individuals who have been impaired by an injury or illness and are considered by their physicians to be medically stable and physically able to begin a comprehensive rehabilitation program consisting of at least three hours of therapy a day, five days a week.
The home health professionals with Gentiva, an affiliate of Kindred at Home, provide medical care and services – including nursing care, wound care and rehabilitation therapies – delivered in the comfort of a patient’s own home. Experienced nurses, therapists and aides work to maximize physical abilities, improve health and well-being and provide essential
education and management of medications and medical conditions.
Our hospice professionals provide a family oriented model of care in order to meet the emotional, physical and spiritual needs of terminally ill patients and their families. Our multidisciplinary services enable patients to stay in a familiar and comfortable environment while receiving pain management, aggressive comfort measures and psychological support to create the best possible end-of-life experience.
Kindred, through RehabCare, offers a variety of rehabilitation services including physical, occupational and speech-language therapies. These services are offered across the entire post-acute continuum through every transition.
