    We provide a cardiac program to help patients recover from conditions such as congestive heart failure, high blood pressure and an array of cardiac procedures. Through this program, our patients benefit from specialized processes, tools and equipment as well as an interdisciplinary team with specific training and extra experience for the conditions they treat.

    Our cardiac program is designed to help patients achieve positive outcomes through evidence-based clinical care so that each patient can regain function – all with the combination of critical thinking and clinical excellence that has made Kindred a leader in the field.

    Interdisciplinary Care Team

    Our patients have access to an interdisciplinary care team consisting of:

    • Cardiologists or other specially trained consulting physicians
    • Nurses specially trained in cardiac interventions and management strategies
    • Rehabilitation therapists (respiratory, physical, occupational and speech)
    • Social workers
    • Registered dietitians
    • Case managers

    Conditions and Services

    We treat a variety of conditions and provide an array of services including, but not limited to:

    • Recovery from:
      • a recent heart attack
      • angioplasty or stent placement
      • pacemaker placement
      • heart surgery
      • heart valve replacement
      • thoracic surgery
      • abdominal surgery
    • Congestive heart failure
    • Angina
    • Arrhythmias
    • Low function capacity hypertension
    • Low cardiac ejection fractions

    Through our cardiac program, patients have access to the following:

    • Specialty physician expertise
    • Physical, occupational and speech therapy available up to seven days a week
    • Pain management
    • Progressive exercise regimen tailored to fit each patient’s needs and abilities
    • Advanced, on-site technology such as point of care PT/INR, EKGs and continuous pulse oximetry
    • Development of a comprehensive cardiac profile to manage risk factors
    • Patient and family education
    • Healthy lifestyle programs such as nutrition education, smoking cessation, stress and weight reduction
    • Individualized plan of care for the patient’s specific needs
    • Ability to reserve a room prior to surgery through our Reservation Program
    • On-site case management

    Recovery and Results

    We continually strive to improve patient outcomes and rates of recovery and to reduce the need for patients to return to the hospital. For patients in our cardiac program, we focus on improving these key areas:

    • Activities of daily living
    • Distance walked during a six-minute walk test
    • Pain management
    • Changes in breathlessness and exertion during exercise

    Easy Referral, Admission and Discharge

    We offer streamlined processes for referral, admission and discharge home. This enables patients to quickly receive the care they need after they leave the hospital and get home faster once they are better. We also feature progressive care planning and education designed to help prepare patients for care at home or transition to a less complex care setting as quickly as their needs allow.
