What is the Paul Diaz Caring Award?

Each year, Kindred Healthcare recognizes caregivers who demonstrate an extraordinary sense of compassion and empathy while caring for their patients or residents. These individuals approach their jobs not just as a means to a paycheck or the completion of their routine duties, but also as an opportunity to make an impact on the lives of those with whom they interact every day. They possess a selfless concern for others, display great respect for patients and family members, and serve as role models for their colleagues and peers.

2017 Winners

Congratulations to Andres “Andy” Regalado, a respiratory therapist at Kindred Hospital Riverside in California;  Annette Jurkiewicz, a home health aide with Kindred at Home Racine in Wisconsin; and Christine “Chris” Palmieri, a physical therapist with Kindred Rehabilitation Services serving the Westminster Village retirement community in West Lafayette, Indiana. These three talented and dedicated caregivers are our 2017 Paul Diaz Caring Award Recipients!

What are the guidelines to be eligible for the Paul Diaz Caring Award?

Candidates with over two years of employment with Kindred may be nominated. Nominations are accepted from Kindred employees, Kindred patients and their families. Prior award winners cannot be re-nominated. Candidates should be employed within Kindred Healthcare.

Nominees may include any Kindred bedside caregiver, including nurses, respiratory therapists, occupational therapists, physical therapists, speech language therapists, therapy assistants/aides, social workers, patient service coordinators, chaplains, medical assistants and, in some cases, supervisors and managers.

Nominations are accepted year-round and must be received by July 9 to be considered for the current year’s award. Nominations received after July 9 will be considered for the following year.

When can I nominate someone for a Caring Award?

Nominations are accepted year round and must be received by July 9 to be considered for the current year award. We will consider nominees for the following year if their nominations arrive after the July 9 deadline.

What is the Paul Diaz Caring Award nomination process?

We give careful consideration to every nomination received. After reviewing the nomination forms or videos, the committee will narrow the pool to the finalists. The finalists must confirm that they can be present to accept the award at the annual Caregiver Summit in Louisville, Kentucky. A committee of leaders from across the Kindred enterprise will then select the award winner from the finalists.

How does Kindred honor Paul Diaz Caring Award winners?

Award winners and a guest are flown to Louisville, Kentucky for the annual Kindred Caregiver Summit. At the summit, the winners will be honored and will receive the Caring Award and a framed certificate. They will also attend a reception in their honor. All travel and accommodations are included. Their names are also engraved on the legacy Paul Diaz Caring Award plaque displayed at the Kindred Support Center in Louisville, Kentucky. Their achievement will be featured in Kindred News and on the Kindred Facebook page. Their photo and story will be displayed on the Caring Awards Wall of Fame for many years to come.

To submit a written nomination, complete the form below and click “Submit.”

For a video nomination, please follow these steps:

  1. Complete the form below and click “Submit.”
  2. Create your video using a smartphone or digital video recorder.
  3. Upload your video to a publicly accessible video hosting site such as YouTube or Vimeo, and email the link to your video to PaulDiazCaringAward@kindred.com. Please include the nominee’s name in the subject line of the email.

The Paul Diaz Caring Award should be awarded to those at facility level; folks who interact with patients in some capacity on a daily or near-daily level. We ask that when considering which members of your team to nominate, please nominate the person(s) that emulates this description:

Each day, we have the extraordinary opportunity to work with talented colleagues who are dedicated to providing compassionate care. We all know those special individuals at our facility or branch who everyone leans on during tough times. Selfless and giving, not only are they always there for our patients and families, but they also serve as pillars of strength when a team member is in need. Day in and day out, they demonstrate the core values of our company and the communities in which we live and work.

Those who deserve praise, but would never expect or ask for it; those individuals who we constantly rely on, but never ask for anything in return.
