Sometimes an injury or chronic illness can make it difficult to attend doctor’s appointments outside the home. When you need to receive care at home, our house call physicians, nurse practitioners, physician assistants and care team provide the compassion and expertise you need to live life to its fullest. 

Our care team provides primary care, specialty care or urgent care under the supervision of board-certified physicians. Our providers are committed to delivering quality care with integrity and dignity. We understand and respect your voice and balance your medical conditions with your personal goals. Complex Chronic Care Management

We understand how difficult it can be to manage a complex, chronic medical condition. That’s why we provide high-quality primary care in the comfort of your home. This allows us to spend more time with you than office-based practitioners. 

We treat a variety of conditions, and tailor a care plan specifically to your individual needs.

We may be able to help if you’re experiencing:

When needed, we also perform medical procedures, such as skin biopsies, vascular access, joint injections, fluid aspirations and more. Through our care, you have full access to world-class specialty physicians who can help make arrangements for these procedures when necessary.

Dementia Care

Our physicians, nurse practitioners, physician’s assistants and staff have extensive experience caring for the unique needs of your loved one living with cognitive impairment and dementia. We use a friendly, integrated care approach that offers state-of-the-art medical therapy, family support and a focus on cognitive, exercise and nutritional therapies.

Hospital Care

We work tirelessly to keep you out of the hospital, but sometimes your condition requires hospital visits or stays. When the hospital level of care becomes necessary, our team collaborates with your physicians at the hospital on each decision regarding your care, every step of the way. 

Our established relationships with hospital partners allow us to provide a seamless transition in your care. We track your progress, when you are likely able to leave the hospital and work to ensure a smooth transition back to wherever you call home.

Care Transitions

When you leave the hospital, there can be uncertainty about your next step in recovery—when to take medications, how often you will need rehabilitation or what to expect in terms of regaining your ability to complete daily tasks. We are here to help you with those transitions in your care. 

When you are ready to leave the hospital, we work with your physicians to determine the best level of care for your condition and set up all needed services including visiting nursing, physical therapy, primary care follow-up visits, and more. We do this because supporting your recovery journey reduces errors in your medication routine, enhances our understanding of your unique medical history and helps you get home faster, when possible, without having to return to the hospital.

Cost-Effective Care

Palliative Care

When you have a chronic illness, you may want help making decisions about how to relieve suffering and improve quality of life. Palliative care can help when you wish to avoid aggressive medical care and shift focus to comfort and symptom relief. These services can be offered at various stages of illness, and you still receive medical treatment alongside a palliative approach.

Palliative care is designed to relieve pain, symptoms and stress of a serious illness. The goal is to provide pain management and psychological support. We do this by keeping open communication between our care team and you and your loved ones so the emphasis is on your needs and wishes.

Podiatric Care

We provide complete podiatric care in your home for a variety of foot ailments. Our routine visits are conducted on a nine-week basis per Medicare guidelines, although exceptions are made for emergencies.

Our care may include:

  • Comprehensive podiatric examinations, to include vascular, musculoskeletal, dermatological and neurological systems
  • Diabetic foot care and coordination of diabetic shoes 
  • Podiatric care for fungal nails, corns and calluses, athlete’s foot and more
  • Relief from ingrown toenails
  • Footwear advice
