Helping You Breathe Easier

By Kindred Healthcare
Helping You Breathe EasierKindred Transitional Care and Rehabilitation Centers bridge the gap between hospital and home. When patients leave a traditional hospital, they often need continued care to recover completely. That’s where we come in.

While all Kindred centers care for patients with pulmonary diagnoses, some centers offer additional and advanced capabilities to care for these patients. Our pulmonary program is designed to help patients achieve positive outcomes through evidence-based clinical care so that each patient can reach his or her highest level of functioning – all with the combination of critical thinking and clinical excellence that has made Kindred a leader in the field.

Our team treats the following conditions:

  • COPD – bronchitis, emphysema, bronchiectasis and/or asthma
  • Post-ventilator stabilization and tracheostomy decannulation and/or stabilization
  • Pneumonia
  • Occupational or environmental lung disease
  • Post-surgical lung resection or transplant
  • Lung cancer
  • Post-thoracic and abdominal surgery
  • Disorders requiring CPAP and BiPAP, including obstructive sleep apnea
  • Obesity hypoventilation syndrome
  • Respiratory infection requiring IV antibiotic therapy

Our interdisciplinary care team is made up of pulmonologists or other specially trained physicians, nurses specially trained in pulmonary care, rehabilitation and occupational therapists, respiratory and speech therapists, social workers and registered dietitians. They utilize specialized processes, tools and equipment to care for our pulmonary patients. The physicians oversee the program, which includes:

  • Anxiety and pain management
  • Individualized plan of care for the patient’s specific needs
  • Healthy lifestyle programs such as nutrition education, smoking cessation, stress and weight reduction
  • Pulmonary hygiene and management strategies
  • Progressive exercise regimen tailored to fit each patient’s needs and abilities
